Registrations received on or after April 13th as well as on-site registration will be accepted, but there is an additional $25.00 late fee.

We are happy to provide a way for facilities to register for their employees to attend the convention without breaking the bank. You can download and print the facility registration form HERE.
Deadline for pre-registration is April 13, 2018. For late or on-site registration add $25.00
